Directions: All current M4 and M5 Biology students should fill in the form below.  The questions ask very basic information about you, and there are also questions that require you to find answers on the pages of this site.  This shouldn’t take you more than a couple of minutes; once you click “submit” all of your answers will show up on a spreadsheet where I can see them.  Make sure that you submit your answers when you have finished, and I will see you in class!
